Please note before you submit:

  • Select your Pathway after a meeting or conversation with your academic advisor or faculty advisor.
  • Reference your degree map or evaluation provided by your academic or faculty advisor when completing this form.
  • A copy of your responses will be emailed to you after the form is submitted. Please save it to your records.
